We encourage team members to schedule a preventive care visit or holistic wellness screening with their primary care provider. <\/p>\n<p><b>These visits may include a wide range of screenings and assessments such as:<\/b><\/p>\n<ul style=\"margin-left:15px;\">\n<li>cancer checks<\/li>\n<li>hearing evaluations<\/li>\n<li>bone density scans<\/li>\n<li>immunizations<\/li>\n<li>OBGYN well-woman exams<\/li>\n<li>blood tests to monitor key wellness indicators<\/li>\n<li>prostate exams<\/li>\n<li>mammograms or thermography<\/li>\n<li>colonoscopies or noninvasive screening option<\/li>\n<li>food allergy testing<\/li>\n<li>vitamin and nutrient deficiency screenings<\/li>\n<li>thyroid function tests<\/li>\n<li>stress or sleep assessments<\/li>\n<\/ul>\n<p>Please submit proof of completion of your visit\/screening to <a href=\"mailto:Wellness@isolvedhcm.com\">Wellness@isolvedhcm.com<\/a>. Tests are not guaranteed to be covered under our benefit plans.<\/i><\/p>\n<h2>*isolved Internal Benefits Knowledge Check<\/h2>\n<p>Complete Course in Learn &#038; Grow \u2013 score at least 80%<\/p>\n<h2>*Monthly Raffle<\/h2>\n<h4>How to Earn Entries<\/h4>\n<ul style=\"margin-left:15px;\">\n<li>Each course completion earns you 1 entry into that month\u2019s drawing.<\/li>\n<li>Annual *preventative screening completion also earns 1 entry.<\/li>\n<li>If you complete multiple courses in the same month, you earn one entry per course for that month.<\/li>\n<ul style=\"padding-bottom:0;\">\n<li>Example: if you complete 3 courses and submit 1 preventative screening in March, that will equal 4 March entries.<\/li>\n<\/ul>\n<li>Maximum number of times you can win the monthly drawing is ONE.<\/li>\n<\/ul>\n<p><i>*Preventative screening does not need to be completed in the month of entry.
