Eligibility

Benefits Premium Discount: Enrolled in any isolved medical plan

Wellness Earning Period

January 1, 2026 – November 30, 2026

Premium Incentive

The 2027 premium discount is $15 per pay period/$30 per month. Discount will be deducted from your premium amount (you will not see a credit on your paystub).

To earn the 2027 premium discount, complete:

• Annual Physical or Wellness Screening (test completion between Jan 1, 2026 – Nov 30, 2026)
• isolved Internal Benefits Knowledge Check (score 80%+)
• Complete 6 of the 7 Learn & Grow Courses

*Annual Physical or Wellness Screening

isolved is committed to supporting the overall health and well-being of our employees. We encourage team members to schedule a preventive care visit or holistic wellness screening with their primary care provider.

These visits may include a wide range of screenings and assessments such as:

• cancer checks
• hearing evaluations
• bone density scans
• immunizations
• OBGYN well-woman exams
• blood tests to monitor key wellness indicators
• prostate exams
• mammograms or thermography
• colonoscopies or noninvasive screening option
• food allergy testing
• vitamin and nutrient deficiency screenings
• thyroid function tests
• stress or sleep assessments

Please submit proof of completion of your visit/screening to Wellness@isolvedhcm.com. We do not provide a form.

Submissions are required to include:

• date of service
• your name
• service/screening provided
• physician or wellness provider

Examples of submission document:

• Doctor’s note
• EOB/billing statement (Explanation of Benefits)

*isolved is not responsible for any fees that may be incurred for your physician to complete your proof of service. Tests are not guaranteed to be covered under our benefit plans.

How to Earn Entries

• Each course completion earns you 1 entry into that month’s drawing.
• Annual *preventative screening completion also earns 1 entry.
• If you complete multiple courses in the same month, you earn one entry per course for that month.
• Example: if you complete 3 courses and submit 1 preventative screening in March, that will equal 4 March entries.
• Maximum number of times you can win the monthly drawing is ONE.

*Preventative screening does not need to be completed in the month of entry. Entry is based on submission date.

Special Completion Drawings

• Once you complete ALL required activities/courses, you qualify for a special Quarterly Completion Drawing + end of the year Annual Drawing.
• These drawings are separate from the monthly drawings.
• Maximum number of times you can win the quarterly drawing is ONE

Winner Notification

Winners will be notified by email and will also be announced to the broader company via a company-wide email.

Q: Can I complete the 8th unrequired course and earn an extra entry into the monthly drawing?
A: Yes

Q: Can I submit more than one preventative screening and earn an extra entry into the monthly drawing?
A: Yes

Q: How many times can I win?
A: You can win only once per drawing (monthly, quarterly, or annual), with the possibility of winning up to three times per year.

Q: If I don’t win the monthly drawing, will my entries carry over to then next month?
A: No

Q: Can I choose the gift I win?
A: No
